Health Forms Information
The State of Wisconsin requires that Health Forms are kept for a period of 3 years, the Potawatomi Area Council uses Campvitals to keep these records. At Conclave, Section G9 will not be collecting Health Forms, all Health Forms are to be done in advance over Camp Vitals. Please find the guide below for the use of Campvitals.
